Italy has been the first European country to introduce social distancing measures, which limited the spread of COVID-19. Furthermore, all the non-life-threatening treatments have been temporarily suspended, including screening programs. This decision leads to unintended effects on the ability to detected neoplasia in their first stages.

Breast cancer is the first cause of death in the European Union among women aged 50-69; in 2017, 8,257 breast cancer cases had been detected thanks to screening activities regularly scheduled by the Italian NHS. We consider possible future outcomes of the ability to detect new breast cancer cases under the social distancing policies based on two alternative scenarios. The reduction in organized screening activities during 2020 will limit the ability to detect no less than 3.43% of the new cases; the economic crisis, caused by the pandemic, will probably reduce voluntary screening as well, increasing the undetected new cases up to 11.73% among the population of women aged 50-69. Cases diagnosed with delay will show up in their advanced stage along with unknown effects on mortality and health care costs. Health care policies should be implemented within the NHS to counterbalance these adverse effects.
